The 2026 MV Hondius cluster, two weeks on

Two weeks after the first WHO Disease Outbreak Notice on the MV Hondius cluster, the picture is clearer but not yet closed. The latest WHO update places the case count at 8 laboratory-confirmed cases, 3 suspected, and 3 deaths — a case fatality rate consistent with historical Andes virus outbreaks in southern South America. The previously suspected case in Germany has been ruled out.

The single most important fact about this outbreak — and the one most worth repeating — is the strain. Andes hantavirus is the only hantavirus that spreads between humans, and only through prolonged close contact. Every other hantavirus strain on Earth requires a rodent intermediary. That is why a cruise-ship cluster matters for Andes virus in a way it would not for Sin Nombre or Puumala.

What we are watching this week:

  • Whether contact tracing among MV Hondius passengers identifies any post-disembark transmission chains.
  • Whether national health authorities in disembarkation countries log additional imported cases beyond those already in the WHO DON.
  • Whether PAHO updates its Southern Cone seasonal alert as the South American autumn case season runs through May–July.

How to read hantavirus coverage

Strain matters

Headlines that say "hantavirus" without naming the strain can mislead. Andes virus (South America) is the only strain that spreads person-to-person. Sin Nombre, Puumala, Hantaan and the rest do not. If an article does not name the strain, assume rodent-only transmission.

Suspected vs confirmed

Early outbreak coverage often blurs "suspected" and "confirmed" cases. Suspected cases meet a clinical or epidemiological definition but have not yet been laboratory-confirmed. Some get confirmed; some get ruled out (as one MV Hondius case in Germany was).

Death rates need a denominator

A 36% case fatality rate sounds catastrophic — and it is, for HPS — but it applies only to confirmed clinical cases sick enough to be tested. Mild or asymptomatic infections are not captured. HFRS strains in Europe and Asia have very different, generally lower CFRs.
